Application can be made electronically on the form below, sending the cheque separately to the address shown on the GBOS application form mentioning that you have applied on-line (you do not need to post a copy of the form itself).
If you wish to apply by post, download the GBOS application form, print it off and send it to the address on the form with payment. The form is in Adobe PDF format, a free Adobe pdf reader can be found on Adobe's website.
